These Ambient AI templates take a conversation between a GP and a patient, and return notes or letters as needed.
Each consultation may be processed several times, using different templates; you can also process your consultation, then record additional detail after the patient has left, and process again if necessary. More information can be found in our guide on using Ambient AI.
These templates return structured notes using the SOAP format, with sections for History, Examination, Diagnosis, and Plan. There are options specifically built to integrate with EMIS and SystmOne and non-integrated default options for those using different clinical systems. The default GP Consultation options include:

This template returns a Referral, with an introduction, reason for the referral, relevant history, and purpose for referral. The Referral will be formatted as a letter with appropriate salutation and valediction.
This template returns a Request for Advice and Guidance letter, including an introduction of the patient and the reason for the request, with key points and medical history summarised. The Request will be formatted as a letter with appropriate salutation and valediction.
This template returns a letter to the patient summarising the reason the visit, the GP’s clinical reason and diagnosis, with explanation in layperson’s terms, and the agreed plan. This will be formatted as a letter with appropriate salutation, closing statements, and valediction.
This template returns a Referral to the Emergency Department, with an introduction of the patient, the reason for the referral, relevant patient history and examination findings, and the purpose of referring the patient. The Referral is formatted as a letter with an appropriate salutation, closing statement, and valediction.
This template returns a summary of the diagnosis and agreed plan. This plan is formatted for use in an SMS message.
This template returns a request for examination with a brief summary of the patient’s condition, history, and the investigation requested. This request is formatted as a letter with an appropriate salutation and closing statement.
This template returns a follow up note using the SOAP format, with sections for History, Examination, Red Flags, Diagnosis, and Plan.
This template returns a letter to expedite the referral, including details of the original referral, clinical reasons for the referral, and rationale for the referral to be expedited. This will be formatted as a letter.
