Ambient AI can transform the way clinicians document consultations, but the quality of the documentation produced depends on the quality of the information provided.

Our AI scribe gives clinicians the option to combine the conversation with existing patient information, supporting documents, automatic language translation, and non-AI coding to produce more complete, accurate documentation.

Clinical context from the outset

Before the consultation begins, supporting information such as referral letters, discharge summaries, PDFs, Word documents, and images can all be attached, allowing the AI scribe to consider information that would otherwise remain outside of the conversation.

 

Include documents provided by patients

Documents provided by patients, including letters, reports and test results, can be added to the consultation, so their contents can be considered alongside the conversation itself.

This reduces the need to summarise other documents or dictate information already available elsewhere.

Automatic language translation

Lexacom automatically translates documents in other languages before they are analysed, allowing clinicians to incorporate important information without relying on external translation tools or manual interpretation.

The translated content becomes part of the overall clinical context available to the AI scribe.

Patient data protected by Patient Shield®

Before any information is processed by AI, Patient Shield® automatically redacts patient identifiable data, helping protect patient privacy without compromising clinical context.

Retrieve patient details automatically

Before the consultation begins, Lexacom automatically retrieves patient information from SystmOne or EMIS.

This is a safety feature to help ensure the consultation starts with the correct patient information. It also adds further context and avoids duplicate data entry.

 

Review and approve coding suggestions

Once the consultation has finished, Lexacom generates structured consultation notes together with coding suggestions.

Unlike AI-generated coding, these suggestions are produced deterministically using Lexacom’s Comprehension Engine®, providing predictable and reliable results.

Where multiple codes could apply, alternative suggestions are presented for review. The clinician remains in control, with nothing added to the patient record until it has been approved.

Insert directly into clinical systems

After the consultation has been reviewed, the documentation can be inserted directly into the patient record using either the clinical system’s API, or Lexacom’s unique injection technology.

Consultation notes are inserted into the appropriate sections of the patient record without copying, pasting, or retyping, helping clinicians complete documentation quickly and maintain existing workflows.
